| Background and Objectives Artificial intelligence (AI) is rapidly entering mental health care, promising to reduce service gaps, yet ethical concerns and human oversight remain essential. This study compared the therapeutic responses of human counsellors and AI systems to examine differences in empathy, personalisation, and relational depth. Method Eight licensed human counsellors and three large language models (ChatGPT-4o, DeepSeek, Grok) responded to five standardised adolescent clinical vignettes (anxiety, ADHD, depression, runaway behaviour, binge-eating disorder). Human participation followed full informed consent and confidentiality protocols; AI prompts emphasised professional boundaries and ethical limitations. Findings Human counsellors excelled in emotional empathy, nuanced affective understanding, and building authentic therapeutic alliances. Their responses were highly individualised and relationally attuned. AI models provided faster replies, broader evidence-based knowledge, and superior diagnostic consistency, but displayed formal, emotionally detached language lacking warmth and spontaneous human connection. Conclusion AI cannot replace the irreplaceable human elements of psychotherapy—particularly the co-creation of a safe, empathic relationship. However, it can effectively serve as an adjunct for screening, psychoeducation, and delivering basic evidence-based skills. The future of adolescent counselling depends on thoughtful human–AI integration that preserves the deeply human core of healing while expanding accessibility and effectiveness. |
